The Spain international has joined Chelsea from Atletico Madrid for a fee in the region of 38 million euros in July 2014.
Diego Costa made 120 appearances across all competitions for Chelsea, scoring 58 goals and supplying 24 assists in return.
The Spain international has been in fantastic form for Chelsea last season.
